Compare Allstate Health Access Plans

Four plans, side by side: Metal Gap, Core, Plus, and Guaranteed Issue. Each pays you a set cash amount for doctor visits, tests, surgery, and hospital stays, no matter what the bill is. There's no deductible to meet first. Amounts are per covered person unless noted.

Based on Allstate's April 2026 Health Access brochure · Updated October 2026

Rates locked for 3 years No annual or lifetime maximum Use any doctor
Please read: this plan provides limited benefits and is not major medical insurance. It pays fixed amounts for covered services, and you pay anything above those amounts. It doesn't meet the Affordable Care Act's definition of Minimum Essential Coverage. In California, South Dakota, and Virginia, it must be bought alongside other health insurance.

Metal Gap

Big hospital payment

A good fit if you want a large one-time payment if you're admitted to the hospital, for example to help with a big deductible on another plan.

  • $6,000 when you're admitted to the hospital (once a year)
  • Surgery pays up to $8,000
  • 4 office visits a year at $100, and 2 urgent care visits at $200
  • $25,000 for an air ambulance

Good to know: each day in the hospital pays just $100. No preventive, lab, X-ray, or chemo benefits.

Core

Everyday care + hospital

A good fit if you want help with doctor visits, checkups, and tests, plus strong hospital benefits.

  • $3,000 a day in the hospital for an illness; $4,000 a day for an injury, rising to $8,000 by year 3
  • 7 office visits a year at $150, plus 2 preventive checkups at $150
  • Pays for X-rays, lab tests, and chemo or radiation ($3,000 a month)
  • Surgery pays up to $9,000

Good to know: checkups have a 90-day wait (30 days in California).

Plus

Richest benefits

A good fit if you want the most from every benefit: more visits, bigger hospital and surgery payments, and cancer treatment.

  • $4,000 a day in the hospital for an illness; $6,000 a day for an injury, rising to $12,000 by year 3
  • 10 office visits a year at $175, plus 2 preventive checkups at $200
  • Chemo or radiation pays $4,000 a month
  • Surgery pays up to $10,000

Good to know: usually the highest premium of the four. Checkups have a 90-day wait.

Guaranteed Issue

No health questions

A good fit if you have health conditions and can't qualify for the other three.

  • No health questions to apply
  • $1,000 a day in the hospital for an illness and $2,000 for an injury, rising by year 3 (up to 90 days)
  • ICU pays $2,000 a day for an illness and $4,000 for an injury
  • 2 office visits a year at $75, plus one X-ray and one lab test

Good to know: no ER, urgent care, preventive, or chemo benefits. Pre-existing conditions still aren't covered for the first 12 months.

Common questions

Questions people ask

Straight answers about how Health Access works. Tap a question to open it.

Is this an ACA (Obamacare) plan?

No. Allstate Health Access is a limited, fixed-benefit medical plan. It pays set dollar amounts when you get covered care, no matter what the provider charges, and you're responsible for any remaining cost. It isn't major medical insurance and doesn't meet ACA standards. If you need that protection, I can quote an ACA plan too.

How do fixed benefits work?

Each covered service has a set payment. For example, Core pays $150 for an office visit. If you use a First Health network doctor, the bill is lowered by the network discount first, so the plan's payment covers more of it. There's no deductible or copay to meet before the plan pays.

Do I need to answer health questions?

For Metal Gap, Core, and Plus, yes. The short application asks about your health, and your answers decide whether you qualify. The Guaranteed Issue plan has no health questions.

Can I see any doctor?

Yes. The plan pays the same amount whichever doctor or hospital you use. First Health network providers give you a discount, which stretches your benefits further, and there's no penalty for going outside the network. Search at firsthealthlbp.com.

Are pre-existing conditions covered?

Not for your first 12 months. That includes conditions you were treated for, got advice about, took medication for, or had symptoms of in the year before your plan starts, and a pregnancy that began before coverage. After 12 months, they're covered like anything else.

Is there a waiting period?

Only for preventive checkups, which have a 90-day wait (30 days in California). Illness and injury benefits start on your first day.

Do my benefits go up over time?

Some do. On Core and Plus, the daily hospital benefit for an injury goes up in your second and third years on the plan. On Guaranteed Issue, both the illness and injury hospital benefits go up. After year 3, they stay at that level.

Will my rates go up?

Your rate is guaranteed for three years on every plan. After that, Allstate can change rates with advance notice. There are also no annual or lifetime maximums on your benefits.

What is the LIFE Association?

Health Access is offered through the LIFE Association, a not-for-profit membership group. You join when you apply (it isn't required in Nevada, Pennsylvania, South Dakota, or Wyoming). Membership includes extras like telemedicine, travel savings, ID theft protection, gym discounts, and help negotiating hospital and imaging bills.

What are the Recuro Health add-ons?

For extra cost, you can add Recuro Health: unlimited $0 virtual urgent care, a prescription benefit with $0 copays for chronic and urgent-care medications (up to $50 per fill), and $0 virtual behavioral health care. Psychiatry visits cost $260 for the first visit and $140 after that. These are services, not insurance, and aren't available on child-only plans.

Does it pay if I have other insurance?

Yes. You get your set benefit even if another plan pays for the same care. Many people use Health Access alongside a major medical plan to help with deductibles and copays.

Can I cover my children?

Yes. Coverage is available for all ages up to 65, and child-only plans are available, except in Missouri, Ohio, and Pennsylvania.
