DentalWise

Dental

A good fit if you want dental coverage on its own, from basic cleanings and fillings up to crowns and implants.

  • Cleanings, exams, and X-rays paid at 100%, with no deductible
  • No waiting period for most services, so you can use it right away
  • Fillings pay 60% the first year and 80% after that
  • Plans 1000 and 2000 add crowns, root canals, and dentures; Plan 2000 adds implants

Dental + vision + hearing

A good fit if you want dental, vision, and hearing all in one plan, especially if you need glasses or hearing aids.

  • The same dental benefits as DentalWise, with a higher top level: up to $3,000 a year
  • Yearly eye exam, plus glasses or contacts, included at every level
  • Yearly hearing exam, plus $750 toward hearing aids every 2 years, including over-the-counter aids
  • Crowns, root canals, and dentures at every level; implants on 2000 and 3000

VisionWise

Vision

A good fit if you only need eye care: a yearly exam, and glasses or contacts.

  • $0 eye exam at a network eye doctor, once a year
  • Standard lenses for a $10 copay, plus $150 toward frames
  • Or $150 toward contact lenses instead
  • No waiting period, and no ID card or claim forms needed in network

How does the dental coverage work?

Preventive care, like exams, cleanings, and X-rays, is paid at 100% with no deductible. For everything else, you pay a $100 deductible each year, then the plan pays a percentage of the cost. Each plan has a yearly maximum it will pay per person: $1,000, $2,000, or $3,000, depending on the plan.

Is there a waiting period?

Not for most services. You can use preventive, basic, and most major dental services right away, and vision and hearing benefits have no waiting period either. The one exception is implants, which have a 12-month wait (6 months in Vermont).

Why do major services pay only 15% the first year?

The coverage for crowns, root canals, dentures, bridges, and extractions steps up over time: 15% in your first policy year, then 50% from year 2 on. Fillings step up too, from 60% to 80%. If you know you'll need major work, it's worth planning the timing with me.

Can I keep my dentist and eye doctor?

Yes, you can see any provider. You'll pay less at a network provider: the National Options PPO 30 network for dental, and the UnitedHealthcare Vision Network for eye care. Out-of-network providers can bill you for the difference between what the plan pays and their full charge.

How do the vision benefits work?

Once each policy year, you get an eye exam ($0 in network) and either glasses or contacts. Standard lenses cost a $10 copay in network, and the plan pays up to $150 toward frames or up to $150 toward contacts. DentalWise Max includes the same vision benefits, but contacts have a $10 copay.

What's an "allowance"?

It's the most the plan will pay for an item, once per policy year. For example, if network frames cost $100, the plan pays the full $100 because it's under the $150 allowance. Out of network, the frame allowance is $75, so you'd pay the remaining $25.

How do the hearing benefits work?

With DentalWise Max, you get a yearly hearing exam paid at 100%, and up to $750 toward hearing aids every 2 years, including over-the-counter aids bought online. Hearing benefits only work through UnitedHealthcare Hearing providers. You can start at uhchearing.com or by calling 1-844-571-4958.

What isn't covered?

Cosmetic dental work, surgical removal of wisdom teeth, jaw (TMJ) treatment, mouthguards, and night guards aren't covered, and braces aren't listed as a covered benefit. For vision, LASIK, treatment of eye disease, non-prescription eyewear, and optional lens extras aren't covered.

Can I replace teeth I lost before the plan started?

Generally no. Dentures or bridges to replace teeth that were already missing before your coverage started aren't covered. Replacing crowns, bridges, or dentures you already had is covered only after you've been on the plan for 12 months. Frequency limits also apply, like one crown per tooth every 5 years.

How do I use it and file claims?

At network dentists and eye doctors, there are no claim forms. The office files for you, and for vision you don't even need an ID card. Out of network, you pay up front and submit for reimbursement. What if I change my mind?

You have 10 days after the policy is issued (30 days in some states) to return it for a full refund of the premium you paid.

How dental benefits are figured

If two treatments would both fix the same problem, the plan pays based on the less expensive one that gives a professionally acceptable result. Out of network, what the plan pays is based on your ZIP Code. Plans with "Plus" in the name use reasonable and customary charges out of network.

What these plans don't cover

  • Cosmetic dental work, whitening, and mouthguards
  • Surgical removal of wisdom teeth; jaw (TMJ) treatment
  • Major services and implants on DentalWise Basic; implants on DentalWise 1000 and Max 1000
  • LASIK and other vision correction surgery; medical treatment of the eyes
  • Hearing care outside the UnitedHealthcare Hearing network

  • Colorado & Minnesota: the DentalWise Basic plan isn't available.
  • Vermont: the implant waiting period is 6 months.
  • Maine: implant benefits are for covered people over age 18.
